Mentorship & Accountability Program (MAP) Application

Thank you for your interest. This is a private, high-caliber cohort led by Dr. Amanda Seay and Dr. Adamo Notarantonio, designed for a select group of dentists committed to elevating their clinical and aesthetic practice at the highest level.

With only 15 spots available, this experience is intentionally curated for individuals who are not only ready to grow, but positioned to extract the maximum value from an intensive, mentorship-driven environment. You do not have to currently be an aesthetic dentist to be accepted but should show high interest and investment in this discipline.

To ensure the right fit on both sides, we ask that you complete the application below. This allows us to build a focused group dynamic and deliver a truly transformative experience for every participant.

Note: Applicants who have attended an imPRES course and/ or purchased the Independent Track will be given priority consideration.

Note: If accepted into the MAP Experience, the amount paid for the Independent Track will be deducted from the Mentorship Track fee.
